This version is written for an external quality-management audit. Adapt the scope and participant instructions before using it internally, and keep any required audit evaluation separate. ### Does satisfaction establish compliance? No. The responses describe participants’ experience. Audit findings, certification decisions, and compliance judgments belong in the appropriate audit process. ### Should everyone rate every stage?
