No. It is an original support-planning questionnaire. There is no validated total score, diagnostic threshold or comparison benchmark. ### Should it collect salary or debt balances? This version does not need those details. It focuses on support topics and access. Do not add private financial fields without a clear justified purpose and suitable handling. ### Does a submitted response request personal help?
