Find the places where readers wanted more, lost the thread, or paused for a memorable scene. This SurveyLegend book feedback template helps turn reactions to one fiction manuscript into focused revision notes.

## When to use this book feedback survey

A book feedback survey collects reactions to a specific manuscript, rather than asking about general reading preferences. Send this questionnaire after beta readers have had time to read the agreed version. The seven optional questions cover reading progress, clarity, pacing, character decisions, and passages worth reviewing. Readers who did not finish can still explain their experience of the portion they read.

## Ask where a reading experience changed

“I liked it” is encouraging, but a chapter reference gives an author something to examine. Ask readers where their attention changed and which passage stayed with them. A slow passage might be confusing, deliberately reflective, or especially moving. The surrounding comment helps distinguish these possibilities.

Miall and Kuiken’s 1994 research on short-story reading examined responses “segment by segment”. The abstract reports associations between stylistically prominent passages, reading time, and readers’ reactions. See the university’s [literary reading research ](https://sites.ualberta.ca/~dmiall/reading/publish.htm)entry for Foregrounding, defamiliarization, and affect: Response to literary stories. These short-story studies support attention to particular passages; they do not validate this questionnaire or tell an author which changes will improve a novel.

Use the responses to build an editorial review list. Preserve what readers found effective as well as investigating unclear transitions. SurveyLegend’s example keeps those observations separate instead of combining them into a single book-quality score.

## Seven book feedback questions

Please give feedback on the fiction manuscript and version named in the invitation. Answer only about the portion you read. Every question is optional, and stopping before the end is useful feedback too. For written answers, use a chapter or scene reference instead of copying a long passage. Do not include personal contact information.

## Adapt the book feedback questionnaire

Name the title, version date, and intended reading portion in the invitation. Use the same version for a feedback round, and ask readers to refer to chapters or scene labels that stay recognizable across formats. For an excerpt, replace “manuscript” with “excerpt” consistently and remove any invitation language that implies the reader has seen the ending.

Tell readers whether you want story-level feedback or sentence edits. This version focuses on the reading experience. Keep line-edit notes in a separate editorial process. Invite readers who reasonably match the intended audience, and record that recruitment approach when reviewing responses.

Ask about a passage without calling it “the exciting opening” or “the confusing middle.” SurveyLegend’s [survey bias ](https://www.surveylegend.com/online-survey/survey-bias/)article explains how wording can steer an answer. Keep room for unexpected reactions with [open-ended questions ](https://www.surveylegend.com/customer-insight/open-ended-survey-questions/), and remind readers that disagreement is useful.

## Printable book feedback questionnaire

Share the printable book feedback questionnaire with readers who prefer to make notes on paper. It includes the same seven optional questions and room for chapter or scene references. Name the manuscript version in the accompanying invitation so the notes can be matched to the right draft.

## Interpret the book feedback responses

Separate responses by how much of the manuscript was read before reviewing clarity and pacing. Do not treat an unfinished reading as a negative rating of the ending. Make a revision table with chapter or scene, reported experience, number of readers mentioning it, and the editorial question to investigate. Keep “hard to follow” separate from “I wanted more detail”; they may call for different changes. When comments conflict, check reading progress and audience fit before choosing a revision. Do not average pace into a quality score: too fast and too slow point in opposite directions. Repeated observations can guide a closer read, but a small invited group does not predict book sales. After revision, collect feedback on the new version separately.

## Frequently asked questions

### Is this a book preference or reading habits survey?

No. This version asks adult readers about one fiction manuscript so an author or editor can plan revisions. Use a separate reading interest questionnaire to choose future titles or genres.

### Can a reader respond without finishing?

Yes. The first question records how much was read. Readers can answer about that portion and skip any item they cannot judge. Keep their observations separate from whole-manuscript feedback.

### Can the answers prove a book is ready to publish?

No. The responses are editorial input from the readers you invited. They are not a validated quality score, a substitute for editing, or a forecast of sales. Review the manuscript alongside the comments before making publication decisions.

In the demo, imagine a fictional well-known performer promoting a reusable bottle; it is not an actual endorsement. Ask how familiar respondents are with the spokesperson, whether the pairing makes sense, what it says about the brand, and what worries them. A response is feedback on this concept, not evidence that anyone has signed a deal or would buy the product. Kamins and Gupta's 1994 experiment varied spokesperson type and product–spokesperson congruence. Its abstract reports more favorable believability and product attitudes under greater congruence; the authors describe the question as “the fit or congruence between image type and the product advertised.” Read the study abstract.
