This salon client survey asks about one hair appointment, from the consultation and waiting time to the finished result. Use the feedback to improve how clients are heard and how the service is reviewed before they leave.

A client may be happy with the haircut but unsure how to ask for a small adjustment. This SurveyLegend questionnaire separates the result from the conversation around it, giving the salon clearer feedback about both the service and the client’s opportunity to speak.

When to use this survey

Send it after the completed appointment, while the consultation and result are still easy to recall. An email survey can give clients time to answer away from the salon chair. Explain how to contact the salon separately for an adjustment or service concern; the questionnaire should not be the only route to help.

Customize your salon client survey

Name the salon and appointment context without requiring a client identifier in the feedback. Keep the focus on hair services; other beauty services need different questions and a separate review of their scope. Explain who reads responses and offer a direct contact route. Do not promise anonymity simply because you removed the name field.

Offer a paper copy at checkout for clients who prefer it, with a return option that does not require answering in front of the stylist. Explain who reviews it and keep it separate from consultation or consent paperwork. Paper responses need their own entry or review process.

How to use the answers

Separate dissatisfaction with the finished result from problems with consultation, waiting or price communication. A client who did not request a change may have been satisfied, or may not have felt comfortable asking; the answer options keep those situations apart. Use open-ended data to identify specific moments the salon can improve rather than reducing every comment to a positive or negative label.

S. Oshima’s 2009 dissertation examined communication in beauty salons in the United States and Japan. The abstract describes a “service-assessment sequence”, in which clients and professionals assess the completed work. This salon communication research informs the question about discussing a change before leaving. It was an interaction study, not validation of this original feedback questionnaire, and service practices can differ across settings.

Frequently asked questions

Is this a salon consultation form?

No. It collects feedback after a hair appointment. Keep service planning, consent and any necessary personal information in the salon’s appropriate separate process.

Should clients answer in front of the stylist?

Give them a practical way to answer privately. Being watched can make it harder to share a concern.

Does a response request an adjustment?

Not automatically. Tell clients how to contact the salon directly, and explain whether and when survey comments are monitored.
