This spa service survey template asks guests about a completed visit, including service explanations, comfort, and whether staff listened to their preferences. Use the feedback to choose a specific improvement for the next guest.

A guest may enjoy the setting but still feel unsure about what happens during a service. This SurveyLegend questionnaire separates those experiences so a single satisfaction rating does not hide a useful detail.

When to use this survey

Send it after the guest has left or offer a private way to respond at checkout. Identify the visit without putting personal appointment details in public reports. If you use an email survey , keep the invitation focused on feedback and explain who will review the answers.

Customize your spa service survey

Name the spa and describe which visit to consider. If you need to compare services, add only a broad service category and avoid collecting sensitive treatment details. Keep “did not need to ask” separate from ease ratings. Explain a separate contact route for guests who need a response; an anonymous feedback form may not let you follow up.

Provide the blank questionnaire with a private return option after the visit. Leave enough space for guests to describe an improvement without writing identifying details. Do not present the form as a treatment record or medical assessment.

How to use the answers

Review each part of the visit separately. For example, unclear explanations need a different response from an uncomfortable room. Group open-ended data by the guest’s actual concern, then choose an owner and a practical change. Overall satisfaction does not establish a treatment’s health effects.

Sivapitak and Sangpikul’s 2024 spa study examined “service quality, service outcomes, and customer satisfaction”. Its spa service research used a convenience sample of 249 Thai customers and reported relationships among those concepts. That distinction informs the separate service-experience and overall-satisfaction questions here. The study does not establish causation, represent every spa market or validate this original questionnaire.

Frequently asked questions

Should this survey ask for medical history?

No. This questionnaire is for visit feedback. Keep any necessary health intake in the spa’s separate, appropriate process.

Can guests ask for a reply?

Provide a separate contact route or an explicitly optional follow-up process. Explain who can see contact details and do not promise anonymity if responses are identifiable.

Is this different from a new-service survey?

Yes. Guests evaluate a service they experienced. New-service research asks people to react to an offer being considered or introduced.
