Cosmetic Survey Template

A single overall rating can hide the difference between an appealing package and a formula someone would actually use. Show one clear concept, state whether respondents have tried it, and ask about each decision in ordinary language. This seven-question cosmetic survey can be adapted and built with SurveyLegend. Each question is optional; the demonstration and printable version must use the same wording and options.

A cosmetic survey is for one named product and audience. It separates how often people use the category from their reaction to texture, scent and packaging. For adjacent needs, explore product testing and name testing .

What this survey helps you decide

Concept and post-use perceptions; avoid skin-health or clinical claims and do not call this safety testing. Learn how a defined audience reacts to one cosmetic concept and what needs improvement before a launch.

How to adapt it

Insert the product type, intended use and actual exposure before asking for feedback. Show scent and texture questions only to people who have sampled the product; for a mockup, ask about expectations instead. Do not infer safety, efficacy or skin compatibility from preference answers.

When to ask

Run a concept version before production, then a separate post-trial version after an appropriate use period. Label the two datasets distinctly.

How to interpret responses

Separate users who handled the product from people who only saw the concept. Compare scent, texture and packaging responses without averaging them into a validated score. Inspect the specific reasons behind uncertain purchase intent before revising a formula or a claim.

Research context

In a study of eight liquid hand soaps with 104 female participants, fragrance affected hedonic judgments in that setting while appearance and transparency did not show the same effect. The product and sample differ from many cosmetics, so this is a cue to ask separately about scent, not a universal rule. Read the Parente et al., Journal of Sensory Studies abstract and its limits before using the finding as context. The questionnaire on this page was written for this use case; it is not a scientifically validated instrument.
