Ask what support people could use without requesting their financial records. This SurveyLegend template explores resource awareness, preferred help, and barriers to participation.

Knowing a resource exists does not mean someone can use it. A useful support survey asks about awareness, practical access and privacy concerns before a coordinator chooses a program.

When to Use This Template

Use this voluntary questionnaire to plan adult workplace or community support. Decide which choices the organizer can realistically investigate. It does not assess financial fitness, decide employment or credit eligibility, or diagnose distress.

How to Adapt the Questionnaire

Name the organizer and describe actual resources before asking about awareness. State who sees responses, how long they are kept and how group findings will be reported. Do not promise anonymity solely because names are absent. SurveyLegend's sensitive questions guidance can help explain the purpose and retain a clear option not to answer. Provide a separate, verified contact route for personal assistance.

When to Ask

Ask before committing to a program format, with a response window that includes different work schedules. If reviewing an existing service, keep participation voluntary and avoid collecting the survey during an individual advice appointment. Repeat only when there is a concrete planning decision to make.

How to Use the Responses

Treat financial pressure as a reported experience, not a diagnosis or a measure of competence. Keep 'prefer not to answer' separate from low need. Compare awareness, sought support, preferred format and access barriers at a sufficiently aggregated level. Privacy concerns may require clearer data handling; scheduling barriers may require another format. Do not infer individual debt, identify vulnerable employees, or claim a program will reduce financial stress. Follow up with a feasible program proposal and explain its limits.

What the Research Adds

Åslund and colleagues' 2014 study abstract describes surveys of 84,263 adults in Sweden examining financial stress and “tangible social support.” The methods identify a cross-sectional design. The findings provide context for considering support alongside pressure, but they do not show that a financial education program causes better health. This template asks about service needs rather than reproducing the study's health or stress measures. Read the financial support study .

Frequently asked questions

Is this a financial well-being scale?

No. It is an original support-planning questionnaire. There is no validated total score, diagnostic threshold or comparison benchmark.

Should it collect salary or debt balances?

This version does not need those details. It focuses on support topics and access. Do not add private financial fields without a clear justified purpose and suitable handling.

Does a submitted response request personal help?

No. Give respondents a separate service route for individual assistance and explain whether and when that route is monitored.
