When to use this end-of-season coaches survey
Send the survey after the season ends, while coaches can still recall specific obstacles and before the next season's arrangements are settled. Invite assistant and volunteer coaches as well as head coaches. Explain who will review their answers; a small coaching staff should not be promised anonymity that the process cannot deliver.

Allen and Shaw's 2013 interview study included eight women performance coaches and two coach managers in two sport organizations. Its abstract identifies “learning and development opportunities” as one theme in coaches' working conditions. That supports asking about development access separately from day-to-day logistics. Keep the club and season in the introduction so respondents do not mix experiences from other teams. If coaches supply their own equipment, change the equipment wording before use or retain the not-needed option. Replace development choices only with realistic opportunities, while keeping an open alternative and a no-support-needed choice.
Avoid asking how much coaches appreciated the club's excellent support. That wording presumes praise. How to interpret your end-of-season coaches survey responses
Review schedule notice and equipment availability first: these identify operational friction the club may be able to change. Report unanswered and not-needed responses separately rather than converting them to low ratings. If peer support was absent, ask the staff privately whether it was wanted before organizing mentoring.
Count the development priorities, then check the written suggestions for a feasible action. A request from one coach may still matter even if it is not the most frequent response. Agree on an owner and review date for one or two changes, and tell coaches what will happen. Do not combine the answers into a coach-performance score or claim that support ratings explain match results.

Is this a survey for players to rate their coach?
No. Coaches complete this questionnaire about the support their club provided. Players' views on explanations, practice feedback, and session delivery belong in a separate coaching feedback survey.
Can volunteer coaches use this template?
Yes. Name the volunteer role and retain choices for support or equipment that was not needed. Do not assume paid and volunteer coaches had the same schedule, resources, or responsibilities.
Should responses affect coaching appointments?
This template is designed to improve support, not select or dismiss coaches. Do not repurpose answers for employment or appointment decisions without a separate, clearly communicated and appropriate process.
