Evaluate one maintenance visit
Use this questionnaire after mowing, pruning, bed maintenance or a seasonal cleanup, once the customer can inspect the result. For feedback on the entire customer relationship, use a broader customer satisfaction survey instead.

Adapt the survey to your setting
Name the visit and the agreed maintenance scope in the invitation, without placing a private address in a public link. Ask someone who actually observed the result, and send the survey while the visit is still easy to recall. Keep “Not sure” and “Not observed” available: a remote property manager may not be able to judge cleanup or noise.
For a recurring contract, use one response per visit rather than combining several visits with different crews or weather conditions. Keep the final open-ended question focused on one improvement. Do not use this survey to approve a contract variation or record an urgent safety incident.

Use the blank paper version when a client prefers to respond after walking through the finished area. Tell the client how to return it and who will review it. Paper answers are not automatically added to online results.

Interpret responses before acting
Read work completion and satisfaction together, but do not combine them into a single score. A partially completed visit may require a scope check, while a fully completed but poorly rated visit may point to communication or cleanup. Keep “No clear scope was agreed” separate from incomplete work: those responses call for different conversations.
Review noise answers only among people able to observe its effect. Do not count “I was not present” as no disruption. Report the number answering each question and group final comments into actionable themes, such as advance notice or debris removal. Check visit records before attributing an issue to a crew, and do not claim that changing equipment will necessarily improve satisfaction.
Frequently asked questions
When should landscaping clients receive this survey?
Send it after a completed maintenance visit when the client has had a chance to inspect the area. Keep the invitation about that visit, not the entire contract.
Can it evaluate a landscape design proposal?
No. Design approval needs different questions about the proposed layout, intended use, budget and requirements. This version concerns work already performed.
Does a low rating prove the crew performed poorly?
No. Weather, agreed scope and what the respondent could observe may affect the answer. Review the specific issue and service records before deciding on a response.
