Customize your coaching session client feedback survey

Keep the survey about one session. Name a specific date if clients attend frequently, and use the same wording when comparing their responses over time. If an employer pays for coaching, explain the actual sharing arrangement before asking for feedback; do not imply that a missing name field guarantees anonymity. Avoid leading questions such as “How helpful was your excellent coach?” Pilot the wording with clients who were not involved in writing it.

A session with a clear goal but rushed pacing needs a different response from a session with comfortable conversation but no useful next step. Discuss a proposed adjustment with the client when follow-up is appropriate under the stated sharing arrangement. Research connection: Graßmann, Schölmerich, and Schermuly (2020; first published online in 2019) describe a “moderate and consistent overall relationship” between working alliance and coaching outcomes in their abstract. No. It collects perceptions of one conversation. Evaluating business outcomes or return on investment needs defined outcomes, relevant costs, an appropriate timeframe, and a stronger evaluation design.

Should clients identify themselves?

Only if a named follow-up serves the stated purpose and clients understand who sees their answers. A private coaching relationship can be recognizable even without a name field.
