People may need to understand a therapy group before deciding whether to explore it. Use seven optional questions to learn what they want explained and which arrangements might work.
This group therapy interest survey helps a counseling service plan clear information and practical access to a proposed adult group. It asks about preferences without collecting symptoms or personal histories. Build the questionnaire in SurveyLegend and keep it separate from clinical assessment, consent and enrollment.
When to Use This Template
Use the template while preparing a group information page or reviewing how the service explains group participation. Describe the proposed purpose, qualified facilitator role and any known practical arrangements before the questions. Include people who are uncertain or not interested; their answers can reveal gaps in information without creating an obligation to attend.

How to Adapt the Questionnaire
Describe only a group the service is genuinely considering and avoid implying that participation is suitable for everyone. Explain who sees feedback and how long it is retained. Apply the principles in SurveyLegend’s discussion of sensitive survey questions when deciding what to collect. Keep symptoms, diagnoses, contact permission and clinical suitability in the service’s separate process. Do not guarantee that other group participants can maintain confidentiality.
When to Ask
Ask before finalizing the group information and timetable, using a voluntary invitation distinct from a care appointment. Give a response deadline and explain whether this is general service planning or feedback on one proposed group. Leave care inquiries to the real service contact route.
How to Use the Responses
Review the clearest information gaps before comparing format preferences. Interest in learning more is not agreement to treatment or evidence of suitability. For Question 6, report the number who answered and note that multiple choices can exceed 100%. Use the final open-ended question to prepare clearer explanations. Do not infer diagnoses, rank respondents or create a waiting list from this survey.
What the Research Adds
In a Wright State University doctoral dissertation, Sarah Peters surveyed 28 lesbian, gay and bisexual college students about barriers to group psychotherapy. The abstract identifies “a lack of knowledge of the process and benefits” as a barrier. This supports asking what people want explained before asking about interest. The small, specific sample does not represent all adults, and the dissertation did not validate this questionnaire. Read the group participation dissertation (2015; degree awarded 2016).
Frequently asked questions
Does this decide who should join a therapy group?
No. It collects information and access preferences. Clinical suitability and informed consent belong to the counseling service's separate professional process.
Does selecting an online format mean online therapy is suitable?
No. A format preference describes practicality, not a clinical recommendation.
Should respondents include personal experiences?
No. The form asks what the service should explain. Keep personal histories and care inquiries out of this planning questionnaire.
