Give the reviewer the basic details of one expense in a consistent order. This reimbursement report records the expense date, business purpose, amount, currency, and receipt status before the organization makes a decision.

When to use this expense reimbursement report

Use this SurveyLegend example to prepare a request for review. Keep one expense in each response so the date and amount refer to the same transaction. The organization's policy determines what supporting information is needed and who can approve payment; submitting the form does not approve a claim.

What the answers help you decide

A clear description of what was purchased and why helps a reviewer understand the request. Separating the amount from the currency also prevents a bare number from being mistaken for a different denomination. Keep receipt collection in the organization's approved channel and use this form to show whether that documentation is available.

Seven questions for your expense reimbursement report

Record one expense you want the organization to review. Every field is optional in this example, but missing details may require follow-up. Use the assigned internal claimant reference. Do not include bank or card numbers, tax identifiers, credentials or receipt contents. Submit supporting documents only through the organization's separately provided process.

Adapt the expense reimbursement report to your audience

Use the organization's real terminology for claimant references and supporting documents. State whether the report covers the full expense or a requested portion. Keep the original currency visible; do not ask people to invent an exchange rate. SurveyLegend’s question-writing guidance can help you define unfamiliar terms so reviewers receive comparable information.

Use one blank sheet per expense and return it through the organization's approved internal channel. Keep receipts separate where the policy requires it and match them using the internal reference. Paper reports need manual review or entry; they do not automatically become approved claims or online responses.

Interpret your expense reimbursement report responses

Review completeness before adding amounts together. Match the claimant reference and receipt through the approved process, resolve unclear partial amounts, and keep different currencies separate. Blank receipt information is a follow-up need, not evidence of fraud. If the goal is broader feedback on travel arrangements rather than an expense request, use SurveyLegend’s business travel survey. Payment, accounting treatment and approval remain outside this form.

Jimmy D. Bravo’s 2019 master’s thesis describes requiring employees to provide “details of their expense claims” through templates (abstract/summary). The disbursement-process thesis examined several changes in one organization. It supports making claim details explicit, but cannot isolate this form’s effect or guarantee faster reimbursement.

Frequently asked questions

Does this report reimburse the expense?

No. It collects details for review. The organization still checks documentation, decides approval and handles any payment in its established process.

Can I attach receipts to this example?

The seven-field example records receipt status only. Provide the organization's approved document-submission route separately rather than placing sensitive receipt details in comments.

Can I combine expenses in several currencies?

Use separate entries and keep the original currency with each amount. Any conversion or accounting decision belongs in the organization's review process.
