A listener’s intention and a speaker’s experience may differ. This SurveyLegend questionnaire asks about one defined conversation so the feedback can lead to a concrete change rather than a broad label about someone’s character.
When to use this survey
Use it after a voluntary listening exercise or a work conversation where both people understand the feedback purpose. For remote conversations, team collaboration tools provide the channel, but this form focuses on the human interaction. Explain who sees the answers before asking for personal feedback.

Customize your listening skills survey
Identify the conversation without recording its confidential substance. Do not add eye-contact requirements or assumptions about one correct communication style. Keep feedback voluntary, especially where the listener has authority over the speaker. This template is for development, not a clinical assessment or a standalone personnel decision.

How to use the answers
Focus on one behavior that the listener can try, such as checking an interpretation before moving on. Group open-ended data by the behavior described, without diagnosing motives. Feeling understood does not require agreement, and one conversation does not establish a person’s overall listening ability.
Kluger and colleagues’ 2024 meta-analytic review examined perceived listening at work and noted that “75% of the literature relied on self-reports”. The workplace listening review supports taking speakers’ experiences seriously while recognizing measurement limits. This original feedback form captures perceptions of one conversation; it is not a validated listening scale or objective performance test.
Frequently asked questions
Is this a hearing or language test?
No. It asks how a speaker experienced a conversation. It does not assess hearing, language proficiency or a clinical condition.
Does listening mean agreeing?
No. Someone can understand a point and still disagree. Keep feedback focused on the conversation rather than whether the listener accepted the speaker’s view.
Should the responses become a personal score?
This version is designed to identify a practice target. It does not provide a validated score or justify a broad judgment about a person.
