When to Use This Template

How to Adapt the Questionnaire

Name the pantry and review period. Publish current contact details and access information beside the invitation, keeping that help available even if someone skips the survey. Check that the actual collection settings match any privacy promise. SurveyLegend's article on anonymous feedback explains why identifying details and follow-up plans need careful consideration. Do not request proof of need in a feedback form.

When to Ask

Run the survey before deciding new opening hours or communications. Avoid relying only on people leaving the pantry: that excludes students unable or unwilling to enter. Keep urgent help separate from the survey review cycle and explain when the team will share its planned changes.

How to Use the Responses

Review users, attempted users and nonusers separately where group sizes permit privacy. A low visit count may reflect lack of awareness, scheduling or no current need; it is not a measure of food insecurity. Compare hours, information clarity and the leading barrier, then select one feasible change to investigate. Retain comments that describe a barrier missing from the options without publishing identifying details.

What the Research Adds

El Zein and colleagues' 2022 interviews with 41 university students distinguished barriers such as “lack of knowledge” and “limited food access.” Read the study . That distinction informs the separate information and access questions here. The study concerned one U.S. university; its themes do not establish how common each barrier is at another campus, and this original questionnaire is not a validated food-security measure.

Frequently asked questions

Does this measure food-insecurity prevalence?

No. It identifies reported pantry-access barriers. A prevalence study needs an appropriate established measure, sampling design and interpretation.

Should students who have never visited answer?

Yes. Their answers may reveal missing information or obstacles that a visitor-only survey would miss.

Can survey completion be required to receive food?

This feedback template is voluntary and separate from access to food. Do not make completion a condition of service.
