When to use this template

Send this seven-question survey after a recent meal or during a short review period. Tying every response to one visit makes results easier to interpret than broad opinions about food service in general.

What this template helps you decide

Use this template for recurring workplace, university, healthcare, residential, or community food service. Review related food templates if the task concerns a restaurant, tasting, preference study, or one catering event.

How to adapt the questions

Replace the setting options and visit window with the real service context. Use neutral question wording , keep observed cleanliness separate from formal safety checks, and add dietary-request instructions outside the feedback survey.

Use a printable version

Use the printable version beside a cafeteria exit or in a dining committee meeting. Provide pencils and a private collection box, and keep forms away from food-preparation surfaces. The downloadable file must be the real blank questionnaire; paper answers are not automatically added to online results.

How to interpret responses

Review each attribute separately before calculating an overall narrative. Compare menu fit, temperature, waiting time, and cleanliness by visit frequency and setting, while protecting small groups. Guak, Oh, and Cho (2022) identified food service factors including “harmonious menu composition and food temperature” as priorities in their mixed-methods institutional foodservice study. Because the work was conducted during COVID-19, it informs what to ask but does not validate this questionnaire or determine priorities in every institution.

Frequently asked questions

Is this the same as a restaurant satisfaction survey?

No. This template is for recurring institutional food service such as a workplace or campus cafeteria. A restaurant survey covers a commercial dining visit and its customer experience.

Can this survey confirm food safety?

No. It collects user observations and satisfaction. Use established inspection, incident-reporting and food-safety processes for hazards or suspected illness.

Should every item be combined into one score?

Not automatically. Review menu fit, temperature, waiting time and cleanliness separately so a strong result in one area does not hide a problem in another.
