Wine Tasting Survey Preview

This wine tasting survey helps an adult group describe coded samples, compare aroma and flavor, and choose which wine they would revisit after tasting.

A favorite wine tells you little about which part of the tasting stood out. SurveyLegend lets a host collect structured impressions alongside one open observation while the samples are still fresh.

When to use this template

Use the survey only for adults at a properly run tasting, after each sample has a neutral code and an agreed serving order. The food evaluation form fits a separate recipe trial. This survey records subjective preference, not wine quality, safety or professional certification.

7 wine tasting survey questions

For the coded sample named by the host, choose one answer per item or skip it. Sample only if you are an adult and choose to participate. Do not include personal health details or tasting notes about another participant.

Customize your wine tasting survey

Replace sample codes with the actual neutral labels, and use one response per sample. For a blind comparison, hide bottle and producer names until all responses are complete; if labels are shown, record that condition. Maintain one serving order or rotate and record it. Do not turn answers into medical or alcohol-consumption profiling; the tasting form covers food samples.

Print a separate copy of the same seven questions for each coded wine. Allow space for the final tasting note, and explain how paper cards will be collected and reviewed; they do not automatically join online responses.

How to use the answers

Compare answers within each coded sample and distinguish aroma intensity from how appealing the flavor felt. Report the number of tasters who evaluated each sample; note label visibility, order and any missing responses. Do not infer that a preference is a laboratory quality measure or that one tasting predicts sales.

Frequently asked questions

Can I compare several wines?

Yes. Give each sample a neutral identifier and collect a separate set of answers for each one. Keep serving conditions recorded.

Should the bottle be visible?

Decide before the tasting. If you want a blind preference comparison, hide brand and label until all sample answers are collected.

Does a high rating establish quality?

No. These questions gather subjective impressions from participants; they are not a laboratory test or certification.
