Make the Questions Fit Your Restaurant
Replace “most recent visit” with a clear visit date if you need to compare shifts. For several locations, add a location question with your actual restaurant names. Keep the core wording consistent across locations you intend to compare.
If guests order at a counter, change the service question to refer to the team who took or handed over the order. Remove table-specific questions from that version. Keep the survey about the experience rather than using questions to advertise menu items.
For takeaway or delivery, replace the dining-area question with a relevant item, such as “How satisfied were you with the condition of your order when it arrived?” Use the same five satisfaction choices and a Not applicable option. Keep delivery and dine-in results separate when the questions describe different experiences. If you ask about drinks, give them their own question so a good meal cannot hide a problem with a drink.
