Invite adults as they leave a named dining hall or canteen service. Identify the location, meal and date before sharing the form, and invite people consistently rather than selecting only satisfied diners. The questions cover a main item and whether anything else was consumed; they do not capture everything a person ate or diagnose nutrition needs. Do not request names, allergies or medical details.
What a Dining-Hall Study Changes
Slavin and colleagues compared a detailed tablet exit survey with a structured dietary interview among 42 adult university students after one lunch service. Their 2019 abstract says the electronic survey “may not be as efficient at capturing the total number of food items consumed.” See the dining-hall study . That is why this short example asks about an additional item and warns against treating a single main-item answer as a complete food record. Unlike the researchers' menu-specific instrument, this original SurveyLegend questionnaire is for service planning, not validated nutrient assessment.

Check Portions Before Changing Recipes
Read the selected main item (Q2) with the reported share eaten (Q3), reason for leaving food (Q5) and portion fit (Q6). Frequent “too large” answers suggest a portion trial; temperature or time answers point to a different operational check. Q4 reminds the team that respondents may have consumed other items. These self-reports cannot calculate nutrient intake, actual plate waste or demand across everyone served. Is this a food diary or nutrition assessment?
No. It captures a few self-reported details from one meal service. A clinical, dietary or nutrient study needs an appropriate validated method and specialist oversight.
Can the team compare breakfast and lunch?
Only if the invitation, item categories and meal labels are consistent enough for the decision. Keep meal windows separate when menus and serving patterns differ.
Does a high percentage of unfinished food prove waste?
No. Respondents estimate how much of one item they ate. Directly measuring waste needs a separate method, and a voluntary survey may not represent every diner.
