How to Adapt the Questionnaire

Name the coalition, purpose, reviewer and expected follow-up in the introduction. Specify whether profiles stay within the coordinating team. Obtain separate permission before adding any information to a public directory or mailing list. Replace collaboration options only with activities the coalition can realistically discuss. When to Ask

Collect profiles before planning discussions and allow representatives time to check with colleagues. Ask organizations to update their information before a new project begins. A profile from last year is not evidence of present capacity or continuing consent to participate.

An organization interested in a shared project but limited by staff time may need a smaller exploratory step. Do not rank organizations as weak partners because they report constraints. Treat open answers as conversation starters, verify details directly, and document any later agreement separately. What the Research Adds

Guo and Acar's 2005 study used survey data from 95 urban charitable organizations to examine “formal types of collaborative activities vis-à-vis informal types.” The abstract reports associations between organizational characteristics and collaboration formality. That distinction supports asking separately about a partnership's purpose and the level of involvement an organization can consider. It does not establish an eligibility score or validate this form.
