How to Adapt the Questionnaire

State the church name, explain the real membership steps and identify the team that handles inquiries. Only offer introduction sessions and contact formats that exist. Keep permission for this inquiry separate from newsletters or a directory. Belief and community affiliation can be personal; follow the guidance on sensitive questions before adding a faith history, family details or other information the initial conversation does not need.

When to Ask

Offer the form when someone seeks membership information, with a clear alternative way to speak to the team. Do not require every visitor to fill it out. Review inquiries on the actual schedule described in the introduction and honor someone’s choice to contact the team themselves.

How to Use the Responses

Use the next-step preference and written question to prepare the reply, then check the selected contact permission before contacting anyone. A request for information is not a membership application, and a blank contact field is not consent to look up a person elsewhere. Summarize recurring questions to improve the membership explanation; do not rank people by perceived commitment or treat inquiry totals as new-member totals.

What the Research Adds

Blacklock’s 2025 doctoral ministry project examined one congregation’s onboarding process with six ministry leaders and ten new members. Its abstract describes “qualitative questionnaires” before and after the intervention and interviews with new members. Read the church onboarding project . The local project supports paying attention to the membership journey, but does not prove that this inquiry form increases membership.
