This nonprofit pledge form template shows how to ask a supporter about an intended future contribution to one campaign. It separates pledge amount and timing from payment, and tells the organization how the supporter wants to be contacted.

A pledge is a statement of intention, not money received. This SurveyLegend example makes the purpose and next step explicit so a nonprofit can follow up respectfully without counting a promise as a donation.

When to use this template

Use it when a named nonprofit has a real follow-up and payment process. If the goal is to understand existing donors’ experience rather than invite a pledge, use donor feedback . Replace the fictional campaign before collecting real responses.

Customize your nonprofit pledge form

Replace the fictional campaign with an approved, specific purpose and a truthful description of how pledges are recorded, revised and followed up. A live pledge form needs an authorized contact method and privacy notice; collect contact details only in the nonprofit’s configured secure flow, not in the public demo. If actual amounts are needed, configure currency and input validation for the intended market; the example uses bands only.

Printable nonprofit pledge form

A printable example can help a nonprofit plan its questions. Do not distribute it as a live pledge instrument until the organization has added its identity, privacy information, contact route and instructions for returning the form. Never put card or bank fields on this survey.

How to use the answers

Keep interest, indicative amount and reminder preference separate. Group open-ended data into information needs and follow-up concerns. Report pledges separately from money received; do not project revenue by adding selected bands or assume an answer creates a contractual obligation.

A 2018 University of Copenhagen field experiment reported that “The firmer the pledge, the more closely the amount donated matches the amount that was pledged”. The pledge research also found that many people declined to pledge. This suggests leaving room for uncertainty and tracking follow-through separately. Its findings do not mean this illustrative questionnaire will increase donations.

Frequently asked questions

Does completing this example make a donation?

No. This is a fictional planning example. A live nonprofit must explain its own pledge and payment processes.

Can a pledge be counted as money raised?

Record intended contributions separately from payments received. The two are different events.

Should the demonstration ask for payment information?

No. Payment must use an appropriate separate process set up by the organization.
