Leaving a volunteer role does not always mean the experience was poor. A project may have ended or someone's available time may have changed. Leave space for those circumstances as well as concerns about the organization.

When to use this volunteer exit survey

Use this template when an adult volunteer finishes or leaves a role. It looks back on the final three months of participation, or the full experience if shorter. SurveyLegend helps gather optional feedback without making an exit conversation a condition of leaving.

To hear from people who are still active, use volunteer satisfaction questions.

How to customize your volunteer exit survey

Send a respectful invitation once the departure is known, and make participation optional. Do not ask people to justify private circumstances. Use the same reference period across replies and keep contact details out of this general feedback questionnaire.

Before sharing, try the questionnaire with a few intended respondents and clarify anything they interpret differently.

Use a survey welcome page in SurveyLegend to explain the purpose, voluntary participation, and who will review the feedback.

Printable volunteer exit survey template

Use this printable volunteer exit survey template as an optional take-home form after a volunteer's final shift. It gives people who prefer paper or are less comfortable online another way to respond. Include the same context, questions, and answer choices as the online version. Tell respondents where to return it and who will review it. Paper answers need separate review or manual entry; they are not automatically added to online responses.

How to use the answers

Separate planned endings and personal circumstances from reported organizational problems. Review clarity, workload, and support together with the final comments. Preserve activities people valued while identifying a concrete coordinator action. The survey describes the people who responded; it cannot explain every departure.

SurveyLegend's guide to organizing open-ended comments can help separate personal circumstances from recurring concerns about the volunteer role.

A volunteer-turnover review examined associations involving support, communication, and volunteers' attitudes. This template turns those themes into separate feedback questions without assuming that any single answer caused someone to leave.

Frequently asked questions

Should every departing volunteer complete it?

Invite them, but keep it optional. Some people will prefer an in-person conversation or no feedback at all.

Keep SurveyLegend's explanation of nonresponse bias in mind when reporting findings: volunteers who reply may have different experiences from those who decline.

Is leaving always a sign of dissatisfaction?

No. Roles end and personal availability changes. Include those possibilities rather than assuming the organization did something wrong.

How is this different from a volunteer satisfaction survey?

An exit survey includes the departure context. A satisfaction survey collects feedback from people currently volunteering and can support earlier improvements.
