Ask About Controls, Not Personal Incidents

Explain who will review responses and give a separate route for urgent reporting or support. This is usability feedback, not a channel for handling a live safety incident.

Why Awareness and Use Need Separate Questions

Bright, Enock, Johansson, Stevens and Margetts studied UK adults' experiences of platform safety technology. Their 2026 abstract states, “Awareness of specific tools is varied.” The safety technology study distinguishes awareness of reporting and blocking from awareness of preventive controls. That supports asking what respondents knew before a task, whether they tried a control and what happened. The study does not validate this original seven-question form or prove the design of any particular platform works.

Adapt the Survey to One Platform

Replace generic safety tasks with controls actually present on the platform being evaluated, without assuming every service has the same menus. Test labels with adult users before launch.
