This meeting evaluation survey template asks attendees whether a meeting had a clear purpose, made room for useful contributions, and ended with clear next steps. Use seven questions to improve the next meeting based on a specific recent experience.

A meeting can finish on time and still leave people unsure what happens next. This SurveyLegend questionnaire separates purpose, participation and follow-through so the organizer can identify a practical change.

When to use this survey

Send it soon after the meeting while the experience is fresh. Name the meeting and date in the invitation. These questions work for in-person, online, or hybrid meetings. For feedback specifically about attending in person, use the in-person meetings survey . If recurring meetings also expose problems with shared work, review the role of team collaboration tools separately from the meeting’s facilitation.

7 questions for your meeting evaluation survey

Answer about the meeting named in the invitation. All questions are optional. Comment on the meeting process rather than rating individual colleagues. Do not include confidential project information or name other attendees.

Customize your meeting evaluation survey

State the meeting being evaluated and avoid combining several meetings into one rating. Keep the no-next-steps option for discussions that did not require action. For remote meetings, add a separate access question only if the organizer can act on the answer. Do not turn process feedback into a ranking of colleagues.

Printable meeting evaluation survey

Offer the same blank questions after an in-person meeting, with a return route that does not require handing comments directly to the facilitator. State who will review the responses and how findings will be shared.

How to use the answers

Compare purpose clarity with relevance and next-step clarity. Group open-ended data into preparation, participation, time use and follow-through. Choose a specific change for the next meeting and evaluate it again; do not add the items into a validated effectiveness score.

Cohen and colleagues’ 2011 study asked working adults about a staff or team meeting they had attended “within 48 hr”. The meeting design study examined how design characteristics related to perceived meeting quality. It informs the focus on a recent, specific meeting, but it does not validate these original questions or prove that any single change will improve performance.

Frequently asked questions

How soon should attendees respond?

Invite feedback while the meeting is still easy to recall. Name the meeting and date so everyone evaluates the same event.

Can this evaluate a recurring meeting series?

Use the same questions after selected meetings and keep the dates separate. A series-level survey needs a clearly stated review period.

Does it produce an effectiveness score?

No. Review each answer and the written examples separately. This original questionnaire has no validated composite score.
