How to customize your AI readiness assessment

Describe the proposed task and the boundaries of the pilot in the introduction. Ask respondents about the same use case, even if their roles differ. Do not ask them to paste confidential data, customer records, passwords, or proprietary examples into the survey. Keep “not sure” choices so gaps remain visible. Use written answers to document the questions that need an accountable owner rather than forcing a premature yes-or-no decision.

Identify what your AI pilot still needs

Separate willingness to try AI from practical preparation. A team can be enthusiastic while still lacking an approved data source, a reviewer, or a way to check results. Record each unresolved dependency, assign an owner, and decide what evidence is needed before proceeding. Compare the pilot with the current way of doing the task. Do not convert a set of positive answers into an automatic readiness score or a claim of compliance.

For example, a team proposing AI-assisted support replies may have a clear task but no approved information source or named reviewer. Record those as unresolved decisions and assign an owner before starting the pilot; enthusiasm does not resolve them.

Who should complete an AI readiness assessment?

Include people who do the task, manage the relevant systems or information, and will review the output. The appropriate group depends on the use case. A leadership-only survey can miss practical dependencies known to the team doing the work.

Does the assessment approve an AI tool?

No. It records preparation questions and perceived gaps. Tool approval, technical evaluation, and any required organizational review remain separate. The questionnaire does not certify a product or authorize a new use of data.

Should we ask about employee confidence?

Confidence can help you plan support, but it does not demonstrate the ability to evaluate an AI output. This template also asks who will review results and what evidence will show whether the pilot is useful.
