An unanswered call does not explain why someone declined. Use seven questions to learn whether the invitation, caller identity, timing or interview format needs attention.

This phone survey participation template helps a research team improve how it invites adults to one planned telephone study. Show a plain-language invitation first, then ask what would make participation practical. SurveyLegend can be used to build the feedback questionnaire; the team must manage recruitment and calls through its own approved process.

When to Use This Template

Use this questionnaire while piloting recruitment materials, before calling the full sample. Include people who would decline a call as well as those willing to join. SurveyLegend’s overview of telephone surveys explains the method; this template addresses the narrower question of how an invitation and call process could work for potential participants.

7 Phone Survey Participation Survey Questions

Please consider the proposed research call described above. These questions ask about your preferences, not permission to call you. Participation is voluntary and every question is optional. Do not enter a phone number, name or health information.

How to Adapt the Questionnaire

Replace the example invitation with the actual study purpose, expected call length, sponsor, verification route and privacy explanation. Offer only invitation and response formats the project can support, and change all copies together. Keep “I would not take part” available. Review wording for leading questions ; do not tell respondents that calls are convenient or trustworthy. Collect any later permission and contact details separately.

When to Ask

Test the questionnaire after people have seen the invitation but before recruitment opens. Use an existing permitted contact route or voluntary pilot panel; do not cold-call people to ask why they dislike cold calls. Give a definite feedback deadline and explain who will review the responses.

How to Use the Responses

Start with purpose clarity, then compare recognition and scheduling preferences. Review barriers among people who say a phone call would not work; averaging their answers with willing participants can hide an access problem. Multiple-choice percentages may exceed 100%. A pilot recruited online will miss some people with limited digital access, and preferences do not predict an achieved response rate. Select one feasible invitation change, test comprehension again and keep refusals separate from failed contact.

What the Research Adds

Peters and colleagues (2023) examined caller ID and text messaging in two waves involving 1,313 surgical patients. Their abstract identifies “getting patients to answer calls from unknown numbers” as a barrier. The study supports paying attention to recognition and contact arrangements, which informs Questions 2–4 here. It used a clinical setting and a comparison with routine hospital calls, not a randomized test of this questionnaire. Its response rates should not be promised for another project. Frequently asked questions

Is this the script for my main telephone study?

No. It gathers feedback on the invitation and call arrangements. Write the substantive interview questions separately for the study's own objective.

Do these answers authorize a research call?

No. Preferences and participation permission are different. Use the project's approved consent and contact process before arranging any call.

Can caller ID prove a call is genuine?

No. Provide an independent verification route. A recognizable displayed number is a cue, not proof of identity.

Provide the actual invitation as an introductory panel, followed by the seven optional questions. Use single-select circles, multi-select squares for Question 2 and a lined area for Question 7. State that no contact permission is collected; include no phone-number field. Paper responses need separate review or manual entry; they are not automatically added to online results.
