What the Questionnaire Covers
The survey anchors responses to one product, task, and four-week period. It then asks about overall satisfaction, ease of common tasks, perceived reliability, the clarity and usefulness of outputs, support effectiveness, and one change. These dimensions help teams choose an area to investigate without pretending one rating explains the entire experience.
How to Customize the Survey
Name the software and define who counts as a recent user. Change the reference period only when the product’s usage cycle requires it, then use the same period when comparing waves. If clarity and usefulness lead to different decisions, split that combined question during editorial review. Link respondents to a separate secure route for account or incident help rather than collecting sensitive details in the survey.
Choose a Useful Reference Period
Four weeks gives regular users time to encounter routine tasks, reliability, information, and support. For infrequently used software, use a longer stated period. Send the survey after a meaningful use window or product change, not only after a support complaint. Keep release cohorts separate if they experienced different versions.
How to Interpret Responses
Start by grouping answers by task, user type, version, and experience level only when those fields are available and privacy-safe. A low ease rating suggests a workflow to observe; low reliability is a user report to compare with technical evidence; low information quality points to content or output review; low support ratings require checking the help route used. Read comments before prioritizing. Satisfaction does not prove productivity, retention, security, accessibility, or technical performance.
Research Note
Petter, DeLone, and McLean’s 2008 qualitative review covered 180 information-systems papers and examined 90 empirical studies through “system quality, information quality, service quality, use, user satisfaction, and net benefits.” Read the review . SurveyLegend uses that distinction to avoid collapsing reliability, information, support, and satisfaction into one vague item. The review covers work published from 1992–2007 and does not validate this original seven-question survey.
Frequently asked questions
Is this the same as a usability test?

Can the ratings identify a software bug?
They can point to a task or pattern worth investigating, but they do not diagnose a defect. Use a separate secure bug-reporting and technical-review process, especially when screenshots, logs, or account details are needed.
Does this survey produce a validated satisfaction score?
No. The questions are an original practical template. Report each item and its response distribution, preserve “Not sure” and non-use answers, and avoid labeling a combined number as a validated benchmark.
