Ask About One Task While It Is Fresh
Name the task, the prototype or product version and the intended goal before the session. Ask the participant to answer immediately after the attempt, while the sequence is still easy to recall. Keep observed behavior, help given and task timing in the moderator’s record instead of asking the questionnaire to reconstruct them.

Van den Haak, De Jong and Schellens compared concurrent and retrospective think-aloud protocols in an experiment using an online library catalogue. They found that “the requirement to think aloud while working had a negative effect on the task performance.” Read the usability study . The result supports separating participant feedback from observed performance and documenting the method, but the single system and think-aloud comparison limit broader conclusions.

Adapt the Survey to the Session
Replace general stage labels with the actual task stages when needed, while preserving a no-uncertainty option. State whether the participant used a prototype or live product, whether a moderator helped and when the survey appears. Remove any question that requests information already captured more accurately by the test system. Test the questionnaire with a colleague before the session.

Print the task name and version at the top without adding participant identifiers unless the research plan requires them. Paper answers require secure handling and manual review; they are not automatically combined with online responses.

How to Use the Answers
Review each task outcome beside the moderator’s observed result. A participant may complete a task yet remain unsure, or report success when the intended state was not reached. Group uncertainty points and open comments by interface location, then examine the underlying recordings or notes. Prioritize repeated blockers and accessibility issues, but keep outliers: a single comment can still reveal a serious problem. Do not total the items into a usability score.
