Website Feedback Survey Template
This website feedback survey template helps you learn why visitors came to your website, whether they completed their task, and what got in the way. Its six questions cover the visit’s purpose, outcome, and difficulties. Adapt the questions to your website and create your survey with SurveyLegend.
When to Use a Website Feedback Survey
Use this example to understand a visit to your website. It focuses on the visitor’s task and experience. If you need to learn who visits your site, use a visitor profile questionnaire instead. Keeping those purposes clear makes it easier to choose the right questions.
6 Website Feedback Questions About the Visitor’s Task
Begin with why the visitor came and whether they completed that task. Then ask about finding and understanding information, followed by two questions about difficulties and improvements. Keep “I am still working on it” separate from “No” when reviewing task completion; an unfinished visit does not yet tell you the final outcome.
A little feedback. A clearer picture.

Customize the Website Feedback Survey
Replace the task options with activities available on your website. Keep an “Other” option so visitors are not forced into a category that does not fit. Ask people after they have had a chance to explore. Decide where and how to offer the survey without interrupting an important task.
For a shop, keep purchasing in the task list; for a service business, include booking only if visitors can book on the site. If comments need page context, add the optional question “Which page were you using when the difficulty occurred?” Ask for a page name, not account details. A focused survey about booking should say “book an appointment” in its questions instead of asking visitors to interpret “your task.”

Find the Page or Process That Needs Attention
Group answers by the visitor’s intended task before comparing outcomes. Visitors seeking support may face different obstacles from people comparing products. Read the explanations from people who answered “Partly” or “No,” then inspect the page or process they describe. Review “I am still working on it” separately. A comment about confusing instructions gives you a specific issue to investigate; it does not establish the cause of every abandoned visit. After making a change, use the same core questions to check whether respondents still report that difficulty.
Frequently asked questions about website feedback surveys
Should I show this survey to every visitor?
Choose a sampling approach that fits your research question and visitor experience. Avoid repeatedly interrupting the same person, and describe the audience reached when reporting results.
Can survey feedback explain every drop-off?
It can suggest reasons, but people who answer may differ from those who leave without responding. Compare feedback with other evidence before deciding what caused a problem.
What is the difference between website feedback and a UX survey?
This questionnaire focuses on a website visit and its outcome. A broader UX survey may cover a product, application, or experience across several interactions. If that wider scope matches your research, explore SurveyLegend’s user experience survey template.
