Post-Purchase Survey Template
Ask recent online buyers about the purchase they just completed. This post-purchase survey focuses on checkout, cost clarity, and confirmation so your team can improve the buying process.
A completed order does not mean every step was easy. Buyers may have worked around a confusing field or an unexpected cost. Ask while the transaction is fresh, before delivery or product use becomes part of the story.
When to use this post-purchase survey
Use this short questionnaire immediately after a completed online purchase. It is for checkout feedback from buyers, not abandoned-cart research or a review of the product after use. SurveyLegend helps you turn recent transaction experiences into a focused improvement list.
To explore why customers chose the product, use the buying motivation questionnaire.

How to customize your post-purchase survey
Keep the timing consistent, such as the order-confirmation stage. For digital products, remove delivery choices that do not apply. If you also want delivery or product feedback, send a separate questionnaire after customers have had that experience. Explain who will review these responses.
Before sharing, try the questionnaire with a few intended respondents and clarify anything they interpret differently.
If the store supports an order-confirmation survey, SurveyLegend's guide to embedding a survey explains how to place it on that page.
Printable post-purchase survey template
Use this printable post-purchase survey template when a buyer discusses a recently completed online checkout with your team. It gives people who prefer paper or are less comfortable online another way to respond. Include the same context, questions, and answer choices as the online version. Tell respondents where to return it and who will review it. Paper answers need separate review or manual entry; they are not automatically added to online responses.

How to use the answers
Review checkout ease alongside the hardest step and open comments. Prioritize repeated, specific problems your team can inspect. Respondents are successful buyers, so their answers cannot explain why all nonbuyers left. Treat missing confirmations as a process to investigate rather than proof that an order failed.
For a practical way to group checkout comments without losing their meaning, see SurveyLegend's guide to organizing open-ended feedback .
A purchase-process study examined product decision-making and transaction experience separately. This template applies that distinction by focusing on checkout, rather than mixing purchase motivation with later product satisfaction.
Frequently asked questions
When should I send a post-purchase survey?
For this version, ask just after checkout while the transaction is fresh. Wait until after delivery or use for questions about those later experiences.
Can this explain abandoned carts?
Not by itself. Only people who completed a purchase can answer this version. Use a separate approach to learn from shoppers who did not buy.
SurveyLegend's explanation of sampling bias helps clarify why feedback from successful buyers cannot stand in for every shopper.
Should I ask for payment information?
No. Ask whether the process worked, not for card details or account credentials. Direct individual order problems to the store's support channel.
