How to Adapt the Questionnaire

State the business unit and quarter with exact dates. Explain what 'delivery' means for the business, such as completed client work or fulfilled orders. Let authorized managers answer only what they know. Review financial records in the appropriate secure process rather than uploading them into the survey. Keep this review separate from staff appraisal.

When to Ask

Run it after the quarter's basic records are available and before the planning meeting. If several managers respond, ask independently first so differences in visibility are not lost in a group answer. Keep the business boundary stable in later rounds.

How to Use the Responses

Look for a pattern that needs verification: demand above plan and frequent late delivery may justify reviewing capacity; low demand and available capacity suggest a different discussion. Neither pattern proves its cause. 'No demand plan' and 'not sure' identify information gaps rather than poor performance. Do not sum the scales, label the business healthy or unhealthy, or predict profit from these responses.

What the Research Adds

Brown and colleagues' 2022 study abstract reports on 220 Western Australian micro, small and medium business owners and examines “efficiency, financial strength, and flexibility in sourcing.” It supports considering several capabilities rather than one overall rating. The pandemic-era study focuses on supply-chain risks; this broader operating review is an original practical adaptation, not a validated version of the study's model. Does this calculate a business health score?

No. Each answer identifies a possible planning question. The template has no validated total, benchmark or pass mark.

Can it replace reviewing financial statements?

No. It only asks how current planning information is. Financial conclusions require the relevant records and qualified review where appropriate.

Who should complete it?

An owner or authorized operating manager with a view of the named business. Several independent responses can reveal differences in knowledge before a planning discussion.
