Read the pattern across goal, role, participation, communication, decisions and support before focusing on one low rating. Compare response spread, not only the average, because a divided team can produce a neutral mean. Code the final answer into one actionable practice, such as role assignment, meeting notes or check-in cadence, and agree who will test the change.
Donelan and Kear's systematic review of 57 papers on online group projects identified “low and uneven participation by students”, lack of clarity and preparation, and poor relationships as recurring challenges. The review supports asking about participation, clarity and support, but it does not validate this form or generalize from higher education to every workplace team. See Donelan and Kear .

Should this form score individual group members?
No. This version focuses on the group's process. Use a separately designed and reviewed peer-assessment process if an institution must make individual grading or employment decisions.
Can responses be anonymous?
They can be, but explain the reporting approach and do not promise anonymity unless the technical and operational process supports it. Small groups may still infer who wrote a comment.
What should happen after the survey?
