Customize your innovation survey

Describe the actual process rather than assuming an official program exists. If the organization has no process yet, use a needs discussion instead of asking employees to rate review criteria they have never seen. Explain what a small trial means in your setting and that normal approval requirements still apply.

Offer a blank copy to employees who do not use a work email account. Provide a private return method and time to answer. Keep descriptions of proprietary ideas in the organization’s designated system, not in this general process-feedback form.

How to use the answers

Distinguish a lack of awareness from an unsatisfactory experience. Group open-ended data by the step where progress stopped: submission, review, feedback or trial authorization. Use the answers to choose a process change. More ideas or more trials do not by themselves prove useful innovation.

Søren Voxted’s 2018 action-research paper examines “the transfer of employees' ideas” into new solutions. The employee innovation study informs this questionnaire’s focus on implementation conditions rather than idea counts alone. Its shop-floor setting differs from many workplaces, and this original process survey does not reproduce or validate a research measure.

Is this an idea submission form?

No. It asks how the process works. Collect the ideas themselves through the organization’s designated channel.

Should employees who proposed no ideas respond?

Yes. Their answers can reveal missing awareness or uncertainty about how to take part. They can skip questions about experiences they did not have.

Can it produce an innovation score?

This version is designed to identify process improvements. It does not provide a validated innovation score or benchmark.
