This procurement survey template asks employees about their most recent purchase request, from finding the right process to receiving useful updates. Use the answers to identify where internal purchasing support needs attention.

A purchase can meet its specification and still leave the requester unsure what happens next. Ask about the steps employees actually experienced before deciding whether the problem is the process, the information, or the support around it.

When to use this procurement survey

Procurement managers and operations teams can use this survey after a request has closed or reached a meaningful review point. It evaluates the internal purchasing experience, not a supplier's commercial performance. Create it in SurveyLegend and name the request period so employees know which experience to describe.

Customize your procurement survey

Keep the questions about one recent request. If some requests are still open, retain the status question and compare those answers separately. Replace general process language with terminology employees recognize, without assuming every request used a digital portal. Ask someone unfamiliar with procurement terminology to try the draft before sending it widely. Review added questions for leading questions so employees can report a difficult purchase without being steered toward a positive answer.

How to use the answers

Separate unfinished requests from completed ones before interpreting timeliness. Look for combinations: unclear instructions may suggest a guidance problem, while clear instructions with poor updates may point to communication. Review comments for specific steps that can be changed and assign an owner to the most practical improvement. Use the approach to open-ended data when grouping comments about instructions, approvals and support. Exclude replies from people who submitted no request in the stated period.

In the abstract, Brandon-Jones (2017) writes: “Processing, Content, Usability, Professionalism, and Training.” The study identifies these as distinct dimensions of e-procurement quality. This template applies that distinction through separate questions about instructions, submitting a request, and receiving help. The original study focused on e-procurement in the UK and Netherlands; these original questions are not its validated scale. Read the e-procurement research for the study's context.

Frequently asked questions

Is this a supplier evaluation?

No. Employees assess the purchasing process and support they receive internally. Supplier quality, delivery and commercial terms need a separate evaluation.

Can employees answer about open requests?

Yes. Keep the status question and interpret open requests separately. Someone waiting for a result cannot fairly judge the completed outcome.

Should this produce a procurement score?

Review each dimension separately. This original questionnaire has no validated overall score, and an average can hide a specific process problem.
