When to use this equipment asset survey

Use this SurveyLegend example during a planned inventory review or when a team reports equipment held at different work locations. Ask respondents to check the item where possible and record only what they can observe. Their answers provide leads for the asset owner to verify; they do not automatically replace the organization's official register.

What the answers help you decide

An item can be present but no longer used, or used regularly but recorded at the wrong site. Keeping location, use and observed condition separate makes those differences visible. Start with one item per response so a missing tag does not become confused with a second device in the same office.

Seven questions for your equipment asset survey

Describe one organization-owned equipment item that you are authorized to report. Every field is optional. Check the item where possible and say when information is unknown. Do not include passwords, software license keys, personal addresses or information stored on the device.

Adapt the equipment asset survey to your audience

Choose one inventory date or review window and give examples of the location codes your team uses. Explain the difference between an asset tag and a manufacturer's serial number before collection. Keep cannot-assess choices for people who can locate equipment but are not qualified to test it. SurveyLegend’s question-writing guidance can help you keep each field focused on one fact.

Give the reviewer one blank form per item and a list of approved site and team codes. Return completed copies to the asset owner for reconciliation. Handwritten entries require separate entry or review and do not automatically update either SurveyLegend responses or an asset register.

Interpret your equipment asset survey responses

Match the asset tag to the register first, then compare location and team information. Put missing or repeated tags in a review queue rather than inventing replacements. Separate an unused spare from a broken item; they require different follow-up. Review the final field for recurring requests using open-ended data guidance. A self-reported working condition is not a safety clearance, and an unreported item is not proof that it has been lost.

Nita Shenoi’s 2006 master’s thesis describes an inventory-status module that would “track the status of the inventory” (abstract). The equipment management thesis illustrates why item status deserves its own field. That project built a separate information system; this SurveyLegend example only collects original inventory-review responses and does not inherit its tracking or notification functions.

Frequently asked questions

Can I list several items on one form?

This example is designed for one item per response. Keeping items separate makes it easier to match tags, locations and follow-up notes.

Does the form scan or locate equipment?

No. Respondents enter the information they know. Any scanning, device discovery or asset-register connection would require separately verified implementation.

Does works as expected mean an item is safe?

No. It records the respondent's observation. Use the organization's appropriate inspection and maintenance process when an item needs a technical check.
