How to use the answers

Review readiness delays separately from defect-report quality. If reproduction information is often missing, agree which evidence the tracking form should request and who can supply it. If verification ownership is unclear, assign that step explicitly. Group open-ended data by the stage where work stalled. These answers do not measure defect rates, test coverage or release safety.

Zimmermann and colleagues’ 2010 study analyzed 466 responses about bug reports and found “an information mismatch between what developers need and what users supply”. Their bug reporting research informs the question about reproduction information. The research involved Apache, Eclipse and Mozilla communities; this original process questionnaire is not the study’s instrument and does not certify software quality.

Frequently asked questions

Can this survey approve a software release?

No. A release decision needs the team’s actual testing evidence and approval process. This questionnaire collects feedback about how the work was done.

Is this a bug-report form?

No. Use the approved issue tracker for individual defects. This survey asks whether reports and handoffs gave colleagues the information they needed.

Can it be used for manufacturing quality assurance?

The questions are written for software testing. Manufacturing needs a different scope, terminology and process-specific review rather than simply changing the title.
