What the Questionnaire Covers

The survey starts with participation level, then asks whether the offer was clear, activities fit the participant’s goal, taking part was feasible, support was helpful, and the program was useful. The final comment asks for one change. These are implementation and experience signals, not proof that the program caused an outcome.

How to Customize the Survey

Name the program and dates, replace “activity, decision, or goal” with the program’s stated purpose, and define support in language participants recognize. Add a separate accessibility or accommodation contact rather than asking for diagnoses. If the audience includes minors or a clinical service, stop and use an approved specialist process. Keep one open response for priorities that fixed choices miss.

Choose a Useful Review Point

Send the survey after participants have enough experience to judge delivery, but soon enough to remember access and support. For long programs, use a midpoint check and a final check with clearly labeled periods. Include non-attenders only when the invitation can ask about access barriers without implying they experienced the activities.

How to Interpret Responses

Segment first by participation level. Low clarity suggests reviewing invitation and orientation materials; low fit suggests comparing promised and delivered activities; difficulty taking part may point to schedule, location, format, cost, or access issues that need follow-up; weak facilitation suggests examining support processes. Read comments before choosing a change. Perceived usefulness is not a clinical, educational, economic, or social impact measure.

Research Note

Proctor and colleagues’ 2011 paper separates “implementation outcomes” from service-system and clinical-treatment outcomes. Read the paper . Its working taxonomy includes acceptability, appropriateness, and feasibility, which supports keeping fit and ease of participation separate from claimed program impact. The paper is rooted in implementation science and mental-health services; it does not validate SurveyLegend’s original questions or establish outcomes for a community program.

Frequently asked questions

Can this survey prove a program works?

No. It records participation and experience. Program effectiveness requires a design that matches the intended outcome, appropriate comparison or baseline evidence, and qualified analysis.

Should non-attenders receive the survey?

They can provide useful access information if the organization has a suitable invitation route. Keep activity-quality questions optional and include “I did not attend” or “Not sure” where needed.

Can the answers be combined into one quality score?

This original template is not a validated scale. Review each item and its comments separately. If a program needs formal measurement, select an appropriate instrument and administration plan with qualified support.
