Make the next sprint easier to work through. This sprint retrospective survey template asks what helped, what got in the way and whether the previous improvement action was tried.

A retrospective works best when the team can discuss specific events, not just a mood score. SurveyLegend helps a facilitator collect seven short observations before the meeting, so quieter voices have a route into the discussion.

When to use this template

Use this after one completed sprint and before its team retrospective. Keep the sprint dates and work visible to everyone answering. For a whole project closeout, use the post-project assessment . For working patterns across several iterations, see the agile assessment .

7 questions for your sprint retrospective survey

Customize your sprint retrospective survey

Name the sprint and dates. Replace options with actual work stages, but keep “no action” and “not sure” where relevant. Share grouped themes before a blame-free discussion. Do not use the questionnaire to rank individual developers. An ongoing weekly status form answers a different question.

Print the same seven optional items for an in-person session, leaving space for one concrete example. Keep responses available to the facilitator, and record meeting actions separately.

How to use the answers

Compare blockers with the actual sprint board, then discuss causes and decide on one small change, an owner and a date to review the result. If the previous action was not tried, find out why before adding another. A perceived improvement is not automatically a measured delivery gain.

Andriyani, Hoda and Amor interviewed 16 practitioners on four agile teams and observed retrospectives. Their university-hosted abstract says teams may not reach all levels of reflection “simply by performing retrospective meetings”. It identifies obstacles, previous actions, underlying reasons and future plans as discussion themes. This supports asking for a concrete event and prior-action follow-through, without claiming the original questions are a validated reflection instrument. Source: Andriyani et al. (2017) .

Frequently asked questions

Should this replace the retrospective meeting?

No. Use answers to focus the team discussion, then agree on a small action and review it later.

Can a facilitator compare sprint scores?

These items have no validated composite score. Compare specific observations and actions while accounting for changes in work and team membership.

What if a previous action was not tried?

Ask what blocked the experiment and decide whether to simplify, defer or replace it before adding another commitment.
