Use the same audit name and dates for everyone. Keep “I did not attend” and similar choices so someone who only helped with preparation does not have to rate the closing meeting. Group open-ended data into actions such as clearer document requests or more time for explanations. Check feedback against the audit plan before changing the process; satisfaction alone does not establish audit quality or compliance.

Lenning, Gremyr, and Raharjo (2022) describe “establishing a co-operative audit atmosphere” in their study abstract. Their single-company study of external ISO 9001 audits links the audit interaction and operational understanding with auditee satisfaction. This template therefore asks about opportunities to explain work separately from the clarity of issues discussed. It is an original feedback questionnaire, not a validated audit-quality measure. Frequently asked questions

Can this survey assess internal audits?

This version is written for an external quality-management audit. Adapt the scope and participant instructions before using it internally, and keep any required audit evaluation separate.

Does satisfaction establish compliance?

No. The responses describe participants’ experience. Audit findings, certification decisions, and compliance judgments belong in the appropriate audit process.

Should everyone rate every stage?

No. Retain the no-experience options and leave questions optional. A participant who missed the closing meeting should not be asked to guess how useful it was.
