Turn the responses into practical improvements

Read task completion alongside visit purpose. Difficulty finding a computer is a different issue from an unavailable book. Compare opening-time feedback across collection periods cautiously, since current visitors cannot fully represent people unable to attend at all. Turn recurring unmet needs into a service-review list. Retain focused open-ended questions to discover needs missing from the current service list.

In the English thesis abstract, Jansdotter and Svensson (2002) write: “tying the purpose of the survey to the goals of the library”. The authors identify this as an area requiring attention in library survey design. This template applies those design principles through a visit-purpose question and partial-success options. Can a university library use this?

A university library usually needs questions about study resources, remote access and academic work. Use an academic-library questionnaire for that purpose.

Does an exit survey include nonusers?

No. Surveying current visitors misses people who do not attend. Use a separate outreach approach if nonuse is part of the question.

Should a name or card number be required?

This service-improvement version does not need either. Keep individual account problems in the library's support process.
