How to customize your learning support survey
Replace the general support wording with the actual service and review period. Adapt language for your learners and provide a separate route for a personal request for help. Do not ask students to disclose diagnoses, grades, or private circumstances just to explain a barrier. Keep “did not need help” separate from “could not get help.” For example, changing the booking explanation may matter more than adding another session if learners do not know how to start. Before sharing widely, try the questions with a few people from the intended audience and clarify anything they interpret differently.

Compare awareness with whether support was used. Among users, review timeliness and whether the explanation clarified the next step; among nonusers, look at the reason for not seeking help. Group improvements into information, scheduling, access, and service delivery. Report feedback as experience of the service, not proof of academic progress.
