Use this school safety survey to ask adult staff about one school's everyday routines, reporting route, supervision-plan clarity and access to help. Seven optional questions point to a practical change without collecting incident reports.

Review Routines, Not Individual Cases

Invite adult staff who have observed the named school during the past four weeks. Tell them which period and routines to consider. Keep existing emergency, safeguarding and incident-reporting channels visible and separate; this questionnaire is not monitored for urgent concerns. Do not ask for student names, alleged conduct or identifying details.

Why One Perspective Is Not the Whole Picture

Ramsey, Spira, Parisi and Rebok (2016) compared school-climate perceptions in a large urban district. Their university-hosted abstract reports that “students reported worse perceptions of safety and connectedness compared to both parent and staff ratings.” Read the school climate study . It supports labeling this as staff feedback and seeking appropriately designed student and parent perspectives separately. It does not validate these original questions or prove a school is safe.

7 School Safety Survey Questions

All seven questions are optional and always visible. Keep the named school and past-four-weeks period consistent. Respondents should use established channels for an active concern.

Make the Routine Examples Local

Replace arrival, transitions and departure with the school's actual routines and vocabulary. Show staff the current supervision and reporting routes before interpreting their answers. If the aim is broad teacher experience rather than safety routines, use SurveyLegend's teacher survey . Do not combine staff, parent and student answers into one undifferentiated safety score.

Find a Routine Worth Reviewing

Compare the selected routine with reporting-route clarity, support access, supervision-plan awareness and comfort raising concerns. A low rating might reflect an unclear plan or uneven implementation; verify it through established school processes before drawing conclusions. Compare staff observations with separately collected student and family perspectives, using appropriate protections. SurveyLegend's education templates hub shows other audience-specific approaches. Never infer from these responses alone that a school is safe or unsafe, and route any specific allegation through the proper channel.

Frequently asked questions

Can this form replace incident reporting?

No. State and provide the school's existing emergency, safeguarding and incident-reporting routes outside the survey.

Should students answer this exact version?

No. It is written for adult staff and asks about staff-observed routines. Student research needs age-appropriate language, consent and safeguarding review.

Does one staff survey measure school safety?

No. It collects reported observations about selected routines. Examine the actual procedures and other stakeholder perspectives before making a decision.
