Classroom Observation Survey Template
This classroom observation template helps a peer observer record what happened during an agreed lesson visit. Separate observations from questions and interpretations, then use the notes to guide a developmental conversation with the teacher.

A useful observation record makes it possible to discuss an actual moment in the lesson. Broad labels such as “engaging” or “effective” leave the teacher guessing what the observer noticed. This SurveyLegend questionnaire asks for concrete evidence and keeps unobserved activities visible.
When to use this survey
Use this form for a voluntary or institutionally agreed developmental peer visit with a defined focus. It is not a hiring, licensing or performance-rating instrument. If you need learners' perspectives rather than an observer's record, review the different purposes of student surveys before choosing a questionnaire.
7 questions for your classroom observation survey
Record only the portion of the lesson you observed. Describe actions and examples without student names or private details. Separate direct observations from interpretations. Every question is optional; use not observed when you lack evidence.

Customize your classroom observation survey
Agree on one focus, such as how learners explain their reasoning, before adapting the prompts. Keep an option for an activity the observer did not see; an activity may have happened outside the visit. Avoid leading questions that presume a teaching method succeeded. If your institution requires an established observation instrument, follow its training and use conditions instead of presenting this informal form as an equivalent.

How to use the answers
Read the observation window first. Group evidence, interpretations and unanswered questions separately; do not convert free-text notes into an improvised performance score. Begin the debrief with a described moment and invite the teacher's context. “Three learners explained their answer” is more discussable than “participation was excellent,” but it still does not show what every learner understood. Agree on a possible adjustment or a future observation focus. Keep the record within the access arrangement and avoid comparisons between teachers based on unlike visits.
Research connection: van der Lans and colleagues (2016) examined reliability using “198 classroom observations” in their study abstract. Their observation reliability study distinguished developmental feedback from higher-stakes decisions and found that repeated visits mattered under its observation design. SurveyLegend applies that caution by recording the visit window and unanswered questions. The study’s visit thresholds belong to its instrument and sample; this informal, unvalidated questionnaire cannot inherit their reliability.
Frequently asked questions
Is this a teacher evaluation rubric?
No. It structures developmental notes from a classroom visit. It contains no calibrated ratings, validated scale or rules for personnel decisions.
What if the observer did not see a learning check?
Record that no check was observed during the visit. Do not conclude that the teacher never checks understanding.
Should learners be named in the notes?
Avoid student names and identifying descriptions. Describe the classroom action needed for the debrief, following the institution's access and privacy procedures.
