When to use this template
Send this seven-question survey before the school finalizes its next family-program calendar. Parents and caregivers can share interests without committing to attend, while the planning team receives a clearer basis for deciding which options to test first.
What this template helps you decide
Use this template when the decision is what to offer next, not whether families are satisfied or already engaged. Review related education templates before choosing a different parent survey.

How to adapt the questions
Replace the example topics with realistic choices and keep the term, delivery formats and notice periods specific. Use neutral question wording . If translated versions are used, review meaning with community members before launch.
Use a printable version
Use the printable version at a school open house, family night or front desk for caregivers who prefer paper. State that it records interest, not registration; provide a private return envelope or collection box and a deadline. The downloadable file must be the real blank questionnaire; paper answers are not automatically added to online results.

How to interpret responses
Start with the number of households interested in each topic, then compare preferred format and time within those groups. Read barrier answers alongside interest: a family can want a program and still be unable to attend under the proposed conditions. Murray et al. (2014) reported that parent involvement barriers “were more commonly reported than facilitators” in interviews with 44 predominantly African American parents of urban public middle-school students. That finding supports asking about access barriers, but it does not generalize to every school or make this questionnaire validated.
Frequently asked questions
Is this the same as a parent engagement survey?
No. This template plans future topics, formats, timing and support. A parent engagement survey examines current school-family participation and communication.
Should families choose more than one topic?
Yes, if the school can compare multiple interests. Keep the list short and include a no-program option so respondents are not forced to express interest.
Does an interest answer count as registration?
No. State that clearly. Use a separate registration process after the school decides which programs it can offer.
