How to adapt it
Use language reviewed for the students' age and local safeguarding requirements. Name the season in the invitation, not through identifying questions. If several teams participate, report only groups large enough to protect privacy. Keep the items about communication and support at the program level; never use one response as an automatic judgment about a coach. Review sensitive questions and open-ended analysis before launch. Keep each item tied to the stated task and avoid collecting data that the decision does not require.

How to interpret responses
Review distributions for practice quality, communication, belonging, support and pressure, then compare only privacy-safe team or season groups. Treat a polarized distribution as a signal for facilitated review even when the average looks acceptable. Code open answers into program-level themes and escalate safety concerns through established processes rather than investigating them in the survey.
A Finnish study followed 824 youth athletes in soccer, ice hockey and basketball and later compared self-reported elite and non-elite groups. The abstract reports that elite athletes had “higher perceptions of a task climate” along with higher enjoyment and autonomous motivation. That association supports asking about learning and team climate, but it does not prove causation or validate this survey. See Zanatta et al. .
Frequently asked questions
Can this survey replace a safeguarding report?
No. Provide separate confidential and urgent reporting routes and explain them before the survey. Do not investigate allegations through a general feedback form.
Should coaches see individual answers?
Decide access through the school's reviewed process. Aggregated reporting can reduce pressure, but small teams may still make respondents identifiable.
Can results be used to rate a coach?
Not automatically. Review patterns with program context, response rates and other evidence. One response should prompt inquiry, not a personnel conclusion.
