This student engagement survey template asks college students about participation in one course over the past two weeks. Explore study activities, opportunities to contribute, and barriers that the course team can address.

Attendance alone cannot explain how a student is engaging with course material. This SurveyLegend questionnaire asks about specific activities and support needs, giving students room to describe what would help them take part.

When to use this survey

Use it during a course while the teaching team can still adjust activities or support. Explain that answering is optional and will not affect grades. For a broader review of facilities, advising and campus life, use a student satisfaction survey instead of expanding this short course check-in.

Customize your student engagement survey

Name one course and keep the two-week reference period. Adjust activity language for online, laboratory or seminar settings without treating spoken participation as the only contribution. Retain “no opportunity” so a course-design limitation is not mistaken for a student’s choice. Have the course team honor the statement that participation will not affect grades.

Printable student engagement survey

Offer a paper version through a return process that does not identify who answered. Keep the same course and date window as the online version. Do not attach student names, IDs or grade records to this informal check-in.

How to use the answers

Look at reported opportunities before judging participation. “No opportunity” and “never” describe different situations. Group open-ended data into activity design, scheduling, clarity and support themes where the comments support them. Share one planned adjustment with the class. These answers do not measure ability, diagnose disengagement or establish why a student received a particular grade.

Indiana University’s National Survey of Student Engagement describes the “multi-dimensional nature of student engagement” and distinguishes areas such as learning with peers and experiences with faculty. Its engagement research framework informs the decision to ask about several aspects of participation. This short, original course check-in is not NSSE, does not use its scoring method and should not be compared with NSSE benchmarks.

Frequently asked questions

Is engagement the same as satisfaction?

No. This survey asks what students do, which opportunities they have, and what gets in the way. Students can like a course while having limited opportunities to participate.

Can these answers be used for grading?

This template is designed for optional course improvement feedback. Keep it separate from grading and honor the introduction’s promise.

Can this produce an NSSE score?

No. It is an original informal questionnaire. It does not reproduce the NSSE instrument, scoring rules or validated indicators.
