Families may know when school starts but still be unsure how to ask a question or where to find a classroom update. A short survey can help you decide what to explain first.

When to use this survey

Use this survey before the first day or during the first week of an elementary class. Name the school year and class, explain that the responses guide group communication, and create the questionnaire in SurveyLegend. Use a separate private conversation for a child’s individual support or records.

Customize your back-to-school parent survey

Replace the channel choices with options your school actually offers. Keep a paper or another workable route for families who cannot use the main digital channel. Review additions using SurveyLegend’s guidance on sensitive questions ; a group orientation survey should not require private family information. Ask a caregiver to try the wording and make sure the explanation can be understood without school jargon.

Provide the blank questionnaire with the same class and school-year introduction as the online version. Explain where to return it and keep it separate from student records. Review the replies through an agreed school process; paper answers are not automatically added to online results.

How to use the answers

Use the most common information gaps to plan a welcome note or orientation session. Keep requests for translated, paper or clearer information visible even when few people choose them. Group open-ended data into general questions you can answer for the class. Do not infer a family’s involvement, a child’s ability or learning needs from a missing response or channel preference.

Petrakos and Lehrer (2011) report in their abstract that “parents often found it difficult to navigate communication with the school.” Their focus-group study concerned kindergarten transition. This template applies that observation by asking whether families know how to contact the teacher and what information would help at the start of school. It is not a child-readiness assessment, and the study does not validate these original questions. Read the school-transition study .

Frequently asked questions

Is this survey for children to complete?

No. This version is addressed to parents and caregivers and focuses on class information. A student-interest questionnaire has a different audience and purpose.

Should families describe health or learning concerns?

Use the school’s established private support route for those conversations. This survey asks about general information and communication preferences.

Can the school choose one channel from the most popular answer?

Use the results as planning input, but keep a workable route for families whose needs differ from the majority. A preference count does not show that every family can access the same channel.
