How to Use the Answers

Review acceptance, access to information, participation and comfort contributing as related but separate signals. Keep small groups from being identifiable, and route specific incidents through the proper reporting process. Lee-Baggley et al. (2026) developed “a 7-item, unidimensional measure of work group inclusion focusing specifically on belongingness.” Read the belonging study . That validated scale is not reproduced here; this original questionnaire has no validated score, and the study’s cross-sectional associations do not prove causation.

Frequently asked questions

Is this a validated belonging scale?

No. These are original practical questions. Use the researchers’ instrument and scoring only under its stated conditions when validated measurement is required.

Should the survey ask for demographic identity?

Not by default. Add identity fields only when they answer a defined question, privacy safeguards are in place and groups will not become identifiable.

Can this replace an incident report?

No. Give employees a separate, clearly explained route for harassment, discrimination, safety or urgent concerns.
