Replace “work area” with the location, team or activity respondents can realistically evaluate. Keep a recent reference period and include “not observed” choices. Follow practical employee surveys guidance: explain confidentiality, avoid asking people to describe an active incident in an open field, and connect results to visible follow-up. How to interpret the responses

Review supervisor, coworker, reporting and training items separately before creating any overall summary. A low reporting-confidence response may need a different action from a low training-relevance response. Schwatka and Rosecrance note that “working with co-workers who value and prioritize safety may be just as important” in a safety climate study . Their construction-industry findings support asking about several levels of commitment, but they do not validate this SurveyLegend questionnaire. Combine survey patterns with incident, hazard and operational evidence.

Is this a safety audit?

No. It captures employee perceptions of everyday norms and support. A safety audit requires qualified inspection of controls, procedures, records and legal requirements.

Should employees report urgent hazards here?

No. Put the established urgent-reporting route in the introduction and reminder text. A general feedback survey may not be monitored quickly enough for immediate risk.

Can responses be compared across teams?

Only when wording, timing and response conditions are comparable and group sizes protect confidentiality. Use differences as prompts for investigation, not as proof that one team is safe or unsafe.
