Use this employee self-evaluation form template to help employees prepare for a focused review conversation. It asks for goals, examples of work, obstacles, and the support needed next. Create the form in SurveyLegend and give employees time to reflect before discussing progress with their manager.

When to use this employee self-evaluation form

Share it before an agreed review meeting and name the period being discussed. Ask employees to describe their contribution with examples rather than rate themselves against vague labels. A self-evaluation adds the employee’s perspective; it does not replace a manager’s preparation or other relevant evidence. Explain who will read the form and how it fits your review process.

How to customize your employee self-evaluation form

Use the employee’s agreed responsibilities and goals as context. Remove prompts that do not fit the role, and make the next-period question realistic for the work ahead. For example, a support employee might describe improving an internal guide rather than claiming responsibility for a company-wide satisfaction change. Avoid requesting confidential customer records as proof. Keep development needs separate from a promise that a course, promotion, or role change will be approved. Before sharing widely, try the questions with a few people from the intended audience and clarify anything they interpret differently.

Printable employee self-evaluation form template

Use this printable employee self-evaluation form template before a face-to-face performance or development meeting. It includes the same questions and space to write, giving people who prefer paper or are less comfortable online another way to take part. SurveyLegend helps you offer both formats. Tell respondents where to return the completed form and who will review it. Paper answers need a separate review or manual entry process; they are not automatically added to online responses.

Prepare a review discussion grounded in examples

Review the stated goals beside the examples and obstacles. Identify where the employee and manager need to clarify expectations, recognize a contribution, or agree on support. Turn the next-period response into a small number of specific actions with owners and check-in dates during the meeting. Do not score writing style or self-confidence as performance, and do not convert the free-text answers into an automatic ranking.

Frequently asked questions

Is this the same as a 360-degree review?

No. This form captures one employee’s reflection. A 360-degree review gathers perspectives from other people as well, using its own purpose and reporting process.

Should employees give themselves a score?

This version uses examples and reflection instead of a numeric score. If your organization requires ratings, define the criteria and explain how they will be used.

What if a goal changed during the review period?

Ask the employee to describe the change and its effect. Discuss the revised expectations rather than treating the original goal as unchanged.
